For 1 go to Dashboard > Settings > Permalinks and then just click save settings.

For 2 providing that you haven't changed any core Suffusion files then all your options will be retained.

This didn't have any effect.

I changed it to "Single line above post" and it fixed the problem. Of course, I'd prefer to have them under the posts. Changing it to "Show in corners above/below content" has the same problem as "Single line below post" (which is what I was using originally).

Is that a Suffusion bug or something else?

It was reported 2 bugs regarded at Comments link and Tags located in the footer of post. I remember that yours bylines is located there, so I make the link

Probably in the next version this bug will be fixed.

In Suffusion. You just applied the workaround - until to new version of theme let the bylines in header.

The new version that just came out doesn't seem to fix the problem.

Actually I noted it in my release notes for 4.2.1 - I wasn't able to replicate the problem, hence I wasn't able to fix it. Can you tell me what exactly your setup is? Specifically, are you using any plugins that tweak post content?
