Feedburner Email Subscription Issue

Hello-
I have a lifestyle blog (www.lifeonsugarberry.com) and I'm trying to add a subscription option, but every time I paste the Feedburner HTML to the Subscription box text area, it ends up displaying 3 subscription boxes rather than one. Any idea what I'm doing wrong?

I've included the HTML below- don't know if that helps:

I only see one Feedburner listed at your site and i checked using I.E. 8 and Firefox.

If you see 3 try clearing your cache and internet history and see if that cures the problem.
